Vacation Bible School (VBS) leaders have permission to photograph/film the minor(s) designated above for any lawful purpose associated with this VBS program. I authorize these photos to be used on social media or website platforms.
I understand that I will be notified in the case of a medical emergency. However, in the event that I cannot be reached, I authorize the calling of a doctor and the providing of necessary medical services in the event that my child is injured or becomes ill. I authorize the leadership of Grace Bible Church to make emergency medical care decisions on behalf of my child, if required by law or a health care provider. I authorize these persons to act in my place to consent to all necessary and appropriate x-ray examinations, anesthetic, medical or surgical diagnosis or treatment, and hospital care. I understand that Grace Bible Church will not be responsible for medical expenses incurred solely on the basis of this authorization. I also understand that the designated adult chaperones reserve the right to restrict my child from any activity that they do not feel is within the physical capabilities of my child.
To create the healthiest environment possible for all students, we ask that you do not bring your children to VBS if they have had any of the following symptoms:
1. Fever or do not appear well
2. Continuously runny nose, regardless of the color of the drainage or treatment with antibiotics
3. Frequent loose or watery stools
4. Frequent cough or sneezing
5. Vomiting
Children should be symptom free for 48 hours before returning to VBS.
If your child has a clear runny nose or other symptoms due to allergies, please let us know. Thank you for doing your part to adhere to this wellness policy.
In order to create a safe and fun environment for all students, we are not able to accommodate children who display violent, aggressive, or extremely disruptive behavior. In the event that a child's behavior becomes unmanageable for VBS staff, we will contact parents/guardians to come pick them up.
We also request that children not bring or use cellphones during VBS.
